Records Request:
In accordance with Texas House Bill 4224 and Texas Health & Safety Code Chapter 181, Trauma to Triumph Counseling and Consulting Services, PLLC provides consumers with the following information regarding access to health records and consumer protections.
Requesting Your Health Records
Clients may request access to their health care records by submitting a written request to Trauma to Triumph Counseling and Consulting Services, PLLC.
Requests can be submitted by:

• Or completing a written records request form upon request
Requests will be processed in accordance with applicable state and federal laws.

Filing a Consumer Complaint
